MAIN FUNCTION
Responsible for conducting biopsychosocial assessments and determining psychiatric diagnosis based on diagnostic criteria from the DSM-5. Initiates establishment of an individualized treatment plan and interacts with other center personnel to communicate information about the patient and the need for treatment.PRINICPLE DUTIES AND STANDARDS OF PERFORMANCE
1. Completes comprehensive assessments of psychiatric and substance abuse history noting onset, course and effect of illness, past treatment and responses, psychosocial risk factors and individual strengths of the patient. 2. Identifies a provisional diagnosis using criteria from theDSM-5. 3.
Identifies any additional treatment recommendations and communicates recommendations to Supervisor. 4. Develops and implements an individualized treatment plan with realistic, measurable goals and objectives. 5. Develops an initial after care plan with identified discharge criteria. 6. Completes clear and concise timely documentation. 7. Demonstrates knowledge of crisis prevention techniques to work with difficult patients. 8. Maintains personal professional growth and development. 9. Displays and demonstrates a positive attitude toward patient, visitors and all facility personnel. 10. Incorporates cultural and age appropriate care into all aspects of communication and service delivery. 11. Maintains confidentiality of all patient information. 12. Complies with facility program and departmental policy and procedures. 13. Performs other duties as directed by Supervisor. 14. Be willing to travel to other locations as needed.POSITION QUALIFICATIONS
Education:
Master's degree in the Mental Health field required. Professional License required.Experience:
Experience in Mental Health services. Thorough knowledge of disease concepts as defined in DSM-5. Experience in the use of Electronic Medical Records System. Demonstrate leadership ability. Ability to communicate effectively both orally and in writing with other employees, program staff, persons receiving service, outside organizations and others.Job Types:
