The Extra Mile is seeking an organized, empathic and resourceful Care Coordinator who can help people navigate behavioral health, healthcare, and community-service systems. This position works with individuals, families, treatment teams, and community partners to connect people with needed services, address barriers, assist with flex-fund requests, and support continuity of care. What You Will Do
- Assess individual needs and develop individualized care coordination plans.
- Connect individuals with behavioral health, physical health, housing, employment, social services, and other recovery supports.
- Assist eligible individuals with the flex-fund process, including explaining requirements, gathering documentation, identifying vendors, submitting complete requests, and following up on approvals and payments.
- Coordinate referrals and transitions between providers and levels of care and follow up to determine whether services were accessed.
- Help identify and resolve barriers that interfere with treatment, stability, and recovery.
- Maintain collaborative relationships with healthcare providers, hospitals, behavioral health programs, schools, justice partners, crisis services, and community organizations.
- Manage multiple individuals, requests, deadlines, and competing priorities while maintaining accurate documentation and timely follow-up. Preferred Qualifications Candidates are encouraged to apply if they meet either of the following:
- Associate or bachelor's degree in health, behavioral health, human services, or a related field OR
- Relevant experience in behavioral health, care coordination, case management, social services services, or a related setting Additional preferred qualifications include:
- Experience working with individuals with mental health and/or substance use disorders.
- Knowledge of community resources and experience coordinating services across healthcare, behavioral health, and social service systems.
- Strong multitasking, organization, communication, engagement, and follow-up skills.
- Practical problem-solving skills, sound judgment, and the ability to seek guidance or escalate concerns when appropriate. Additional Requirements
- Minimal travel.
- Valid driver's license
- Employment is contingent upon successful completion of required background, exclusion, and pre-employment drug screenings.
Pay:
$40,000.00 per year
