Southside Community Health Services
Minneapolis, Minnesota Support Whole-Person Care—Join Southside Community Health Services as an Integrated Care Coordinator Southside Community Health Services (SCHS) is seeking an Integrated Care Coordinator to support patients in accessing comprehensive, coordinated healthcare services. This role plays a key part in advancing integrated care by connecting patients with medical, behavioral health, and community resources to improve overall health outcomes. About the Role The Integrated Care Coordinator works closely with patients and care teams to assess needs, coordinate services, and remove barriers to care. This position supports whole-person, patient-centered care by facilitating communication across providers and ensuring patients receive the resources and follow-up needed to achieve their health goals. Key Responsibilities
- Assess patient needs and coordinate care across medical, behavioral health, and community services
- Support care planning and follow-up to ensure continuity and quality of care
- Connect patients with internal and external resources, including social services and community programs
- Collaborate with interdisciplinary care teams to support integrated, patient-centered care
- Assist patients in navigating the healthcare system, including referrals and appointment coordination
- Maintain accurate and timely documentation in the electronic health record
- Participate in team meetings, case reviews, and quality improvement activities
- Support patients in overcoming barriers to care, including transportation, financial, and social challenges
- Assist with additional duties as assigned to support clinic operations What You Bring

- Commitment to the SCHS mission and values Qualifications
- Bachelor's degree in social work, healthcare, or related field (or equivalent experience)
- Experience in care coordination, case management, or community health preferred
- Knowledge of community resources and social service systems
- Experience with electronic health records (EPIC preferred)
- Bilingual (English/Spanish or Somali) preferred Job Information
