Job Description

We are seeking a highly organized and detail-oriented Medical Assistant (with Clinical Experience) or LPN to join our team as a Care Coordinator. Premier Health offers applicable in office training. As a Care Coordinator, you will play a crucial role in providing support and coordinating care for our patients. This is an excellent opportunity for someone with clinical experience and a passion for helping others.
Duties:
  • Call clients monthly, coordinate patient care, maintain BP logs and monitor maintenance, develop and execute care plans to counsel and effectuate positive health and wellness goals.
  • Maintain accurate and up-to-date patient records, including medical history, treatment and care plans
  • Answer phone calls and respond to inquiries from patients, providers, and other healthcare professionals
  • Coordinate referrals to specialists and follow up on patient care
  • Collaborate with the healthcare team to ensure continuity of care for patients along the healthcare continuum.
  • Provide general administrative support, including filing, data entry, and correspondence
Skills:
  • Proficiency in office software applications (e.g., Microsoft Office Suite) and EMR applications
  • Knowledge of medical terminology and procedures
  • Experience working in a medical office setting preferred
  • Strong organizational skills with the ability to multitask and prioritize effectively
  • Excellent communication skills, both verbal and written
