Job Description
Overview We are seeking a dedicated and organized Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that patients receive the highest level of care by facilitating communication between healthcare providers, patients, and their families. This position requires a strong understanding of medical terminology and administrative processes within a medical or dental office setting.

Hybrid (50% work from home, 50% field visits within designated areas in King and Snohomish County)Pay:

Part-time.Duties:
Schedule and coordinate patient appointments to maximize provider efficiency. Maintain accurate, up-to-date patient records. Communicate appointment reminders, follow-up care, and test results to patients. Assist with insurance verification and pre-authorization. Collaborate with healthcare providers for seamless care coordination. Provide administrative support to the healthcare team. Organize and facilitate care activities for a holistic health approach. Engage clients through community-based, face-to-face encounters and in-home visits. Conduct comprehensive health screenings and develop individualized Health Action Plans. Support clients in goal setting, care planning, and managing their own health. Promote client engagement in work, volunteer activities, and social support networks. Assist with mental health and substance use treatment as needed.Travel Requirements:
Travel to client/patient homes within designated areas. Access to reliable transportation with mileage reimbursement provided. Valid Washington State Driver's License required.Experience & Qualifications:
Care Coordinator with previous experience in Health Home program highly preferred. Bachelor's degree in healthcare, behavioral science, social services, or related field; LPN, Peer counsellors. Certified Medical Assistants with an associate degree Familiarity with medical terminology and electronic medical record (EMR) systems. Strong organizational, communication, and interpersonal skills. Proficiency in MS Office (Word, Excel, Outlook). Ability to multitask and work in a fast-paced environment. Demonstrated knowledge or interest in behavioral and physical health issues. Current First Aid & CPR certification or ability to obtain within 90 days of hire. Bilingual/bicultural skills are encouraged. Experience in social work or related fields is preferred.
