Full Job Description Essential Duties and Responsibilities:
Complete medical and/or behavioral health assessments that are typically routine using standardized tools and criteria in accordance with contract requirements; escalate complex or non-standard cases that fall outside defined protocols. Conduct on-site, video, and/or telephonic assessments based on program requirements to determine an individual's needs for services and supports, eligibility, and level of care based on established guidelines, seeking guidance from senior staff as needed. Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies; ensure accurate and consistent documentation of assessment outcomes. Travel to client, consumer or program locations to perform in-person assessments exceeding 50% of the workweek in support of program delivery. Minimum Requirements High school diploma or GED required and 2+ years of relevant professional experience required, or equivalent combination of education and experience. Travel on-site to client, consumer or program locations for more than 50% per workweek required; specific travel requirements based on program contract requirements. Bachelor's degree in health, social work, or a related field At least two (2) years of experience in the human services field Ability to assess individual needs effectively and provide accurate counseling regarding available support options Strong multitasking skills with the ability to prioritize and manage a high volume of tasks Proficiency in Microsoft Office Suite (Word, Outlook, Excel) and comfortable working with various digital tools Ability to work independently and as part of a team in a fast-paced environment Strong written and verbal communication skills, with the ability to interact professionally with internal teams and external providers Willingness to travel Ability to perform the physical demands of the role, which may include standing, walking, bending, lifting, carrying, and transporting materials as needed.
Preferred Requirements:
Experience working with Medicaid Waiver, Long-Term Services, and Supports, or similar programs Knowledge of state-specific regulations related to Medicaid Waiver and other support services Familiarity with Person First standards and HIPAA compliance in healthcare settings Additional certifications related to social work, healthcare, or human services are a plus
