The Impact You'll Make As a Client Care Coordinator you'll be the central point of contact for clients and their families, ensuring their needs are met with compassion and efficiency. You'll coordinate services, assist managing schedules, and act as a trusted advocate—helping clients feel confident and cared for every step of the way. What You'll Do Serve as the primary liaison between clients, families, and care teams. Coordinate relationships between caregivers and our clients, ensuring timely and accurate delivery of care. Maintain up-to-date client records in our care management system. Conduct regular check-ins with clients to assess satisfaction and address concerns. Collaborate with caregivers, and administrative teams to ensure seamless care transitions. Assist with training new hires in clients' homes/facilities and serve as fill-in support for call outs. Provide empathetic support and problem-solving for client inquiries or issues. On call rotation for every other week/weekend. What You'll Bring Certified CNA preferred; previous experience in client services, care coordination, or healthcare administration is a plus. Strong interpersonal and communication skills, with a genuine passion for helping others. Proficiency with scheduling and willingness to learn our scheduling system, with open availability to work any shifts if needed. Excellent organizational skills and the ability to manage multiple priorities. A calm, solutions-focused approach in high-pressure situations. Knowledge of healthcare or home care services is a plus.
