At Wellbeing Alliance, we believe the people providing care deserve care too. Not in a surface-level way, but in how we structure schedules, create space for development, build community, and protect sustainability. When the people doing the work feel supported, the work gets stronger. We are a DBHDS-licensed provider of Mental Health IOP services serving Richmond, VA and surrounding counties. Many of our clients have experienced homelessness, grief, trauma, hospitalization, or major loss. They need someone who walks alongside them and helps them build a path forward. We're looking for a Care Coordinator who wants to do this work with intention: with structure, follow-through, and pride in the standard of care. What This Role Includes Care Coordination Part-time | Daytime Hours | Caseload-Based Care coordination here is active, hands-on work that happens throughout the week. You stay in regular contact with each client: calling, checking in, meeting in person, and following through as needs come up. You connect each client's care so appointments get made and kept, providers stay in communication, and every client knows their next step.
You will:
Meet each new client early on for a relaxed, hour-long conversation over tea, breakfast, or lunch, using our care coordination model, "The Next Best Thing": where they are now, what's in their way, what a hard stretch looks like for them, and what they're working toward Stay in contact with clients multiple times a week through calls, check-ins, and in-person support, plus a scheduled one-on-one each week to review progress and plan ahead Identify needs beyond treatment, such as housing, employment, food, benefits, transportation, and family supports, and connect clients and their support people to referrals Coordinate with each client's primary care doctor, psychiatric provider, outside therapist, and any substance use providers so everyone's plan lines up Connect every client to their psychiatric evaluation and ongoing medication management Schedule appointments, help with paperwork, follow up afterward, and make sure clients know what's next Respond quickly when the care team flags a need Plan discharge from day one alongside the Clinical Lead and Program Manager Complete timely documentation aligned with DMAS standards; your notes support every continued stay request Attend our weekly care team meeting This is relationship-driven work built on consistency, professionalism, and follow-through. Compensation $300 per month for each client on your caseload, prorated for partial months. Your caseload grows as the program grows. What We're Looking For Required:
Virginia QMHP-A or QMHP-Trainee registration, or higher Experience in case management or care coordination Knowledge of Richmond-area resources: benefits, housing, transportation, and healthcare Strong, timely documentation skills Valid driver's license Professional liability insurance
