Full Job Description Hours :

40 hours per week; Monday - Friday, 8:30 am to 5:00 pm As a Clinician I - ICM (Intensive Case Management), you will: Provide case management and behavioral health services for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team. Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in conjunction with the ICM treatment team. Provide individual supportive counseling and symptom management. Conduct comprehensive assessments of psychiatric mental status, diagnosis, and family structure and relationships Provide treatment, rehabilitation, and support services. Provide direct clinical services to clients on an individual, group or family basis in the office and in community settings. Act as liaison to and consult with community agencies and families to coordinate treatment.

Intensive Case Management (ICM) is a community-based program serving adults living with serious mental illness who require a higher level of support to achieve stability and independence. The program focuses on individuals with complex needs who may be at risk of hospitalization, homelessness, or disengagement from care. ICM utilizes a multidisciplinary, team-based approach to provide comprehensive, individualized services in the community. Staff work closely with clients to coordinate care, support access to housing and benefits, assist with medication and treatment engagement, and build skills to promote recovery and independence. With smaller caseloads and a strong emphasis on outreach, ICM allows staff to develop meaningful relationships with clients while delivering high-quality, person-centered care. Licensure :

Two years of experience providing behavioral health services. Demonstrates knowledge of clinical and rehabilitation theory. Knowledge of substance use disorders and addiction preferred.
