Dismas Home of New Hampshire is a residential treatment and reentry program that provides justice-involved women, including women veterans, with substance use disorders a safe, home-like environment to heal, recover, and rebuild their lives.

Position Description:

A Clinical Case Manager is a key part of the Dismas Home of New Hampshire Team, supporting an

ASAM 3.1

residential treatment program serving women with primary substance use disorders, including women involved in the criminal justice system and those with histories of trauma. The Clinical Case Manager will provide individual case management, group facilitation, recovery support, care coordination, and residential program support within a structured, trauma-informed environment.

Primary Responsibilities include the following:

Provide individual case management and assist clients with treatment goals, resources, housing, employment, legal obligations, healthcare, and transition planning. Facilitate substance use, recovery, relapse prevention, life skills, and other therapeutic/educational groups. Maintain timely and accurate clinical documentation. Support a safe, structured, and recovery-oriented residential environment. Conduct room and chore checks and reinforce program rules and expectations. Conduct urinalysis/drug testing according to program policies. Complete medication logging, auditing, and observation responsibilities as required. Provide recovery support and accountability throughout the program day. Provide transportation to outside services, appointments, and recovery activities as needed. Collaborate with the clinical team, criminal justice partners, and community providers as appropriate. Utilize trauma-informed, person-centered, and recovery-oriented practices. Opportunity to carry a small counseling caseload under the direct supervision of a LADC, based on qualifications, credentialing, and demonstrated competency. Skills and Abilities:

Ability to assist clients with developing short and long-term goals related to recovery, sobriety, case management, and personal life goals. Ability to complete documentation/shift notes, and tasks related to guidelines of Dismas Home policies Ability to work collaboratively with other Dismas Home of New Hampshire team members Ability to consistently exercise sound judgement in a crisis situation Strong Verbal and Written Communication Skills Proficient Computer Skills, including basic typing skills

Strong Interpersonal Skills Qualifications:

Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Behavioral Health, or related field. Must be under LADC supervision or willing and eligible to work toward LADC credentialing. Valid driver's license required. Strong communication, organization, documentation, and interpersonal skills. Ability to maintain professional boundaries while balancing clinical and residential responsibilities. Experience with substance use treatment, residential services, trauma-informed care, women's services, or criminal justice populations preferred. Pay:

$59,000.00 - $64,000.00 per year
