Nurse Practitioners with home health experience are needed to perform risk assessments for Medicare Advantage plan members. This per diem opportunity offers a flexible schedule within Hillsborough County, potentially including locations such as Manchester, Nashua, and Bedford. The primary responsibility will be to complete in-home risk assessments, typically lasting about 45 minutes, with the nature of the visits ranging from annual health evaluations to post-discharge follow-ups. You will have the flexibility to create your own schedule and choose your shift location within the coverage area.

Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Evaluate medical histories, current medical conditions, and review medication history Educate patients on their health conditions and medications Identify care gaps and suggest recommendations for resolving them No requirements to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in New Hampshire Current CPR certification (BLS) Board-certified through AANP or ANCC Preferred minimum of 6 months of Nurse Practitioner experience MSN degree from an accredited nursing program Compensation for this role is $80$90 per completed evaluation, in addition to a mileage stipend and pay for administrative time.
