  • Position Summary:
  • This non-exempt analytical position is based in a large outpatient family medicine practice; data coordinator provides support to clinical teams to ensure that patients receive chronic disease care and preventive care consistent with evidence-based guidelines.
Care not delivered according to guidelines are considered "gaps". Data coordinator also performs and coordinates patient outreach to improve patient's health. Directly supports operational efforts for all population management initiatives that involve quality metrics, assists practice teams in quality and process improvement initiatives. Utilize electronic registries such as Regional Health Information Systems, Arcadia Analytics, and eRecord dashboards for comprehensive patient care management.
  • Salary Range:
  • $19.50 - $25.00 an hour _The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations._

Education:
B.S. or B.A. degree preferred; or equivalent experience. AAS required. 2.
Experience:
3 years in a professional office environment; 2+ years clinical, operational, quality data collection and reporting experience. Experience with LEAN processes and/or process improvement methods highly desirable. 3.
License/Certification Required:
None 4.
Skills:
Basic MS outlook, Word and intermediate MS Excel are required. Excellent communication skills (verbal and written) required for interactions with patients and practice staff and telephone communication. 5. Computer database navigation
