Job Description
The following are some of the duties and responsibilities of the CHW and may require travel to other Stamford Health sites and clinics: Educate patients about relevant community resources appropriate to their needs. Help patients set personal health related goals. Assist patients access transportation for appointments. Navigate patients through the care continuum including appointment scheduling, visit completion, follow-up coordination and verification with the physician that the patient understood the results of the visit. Provide educational opportunities via lectures on health-related topics. Collaborate on strategy and program roll-out, report on barriers and successes of the program to the CHW program coordinator/manager. Complete Spanish interpreter certification. Provide language translations at the hospital and in the community to help access medical resources. Assist community members find services such as rent relief, housing, healthy food and energy assistance. Assist community members with applications for FAP or Medicaid
