Week Job Duties:

Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible. Assist clients in developing and understanding care plans. Document activities, care plans, and results in an effective manner while strictly adhering to the policies and procedures in place. • Work collaboratively and effectively within a team. Facilitate communication and coordinate services between providers. Contact disengaged attributed clients meeting face to face in the home or community. Assist newly engaged/re-engaged clients in accessing services, when appropriate, and follow up on requested services. Completing follow-up face to face tasks as needed. Coach clients in effective management of their chronic health conditions including mental health and substance use disorder. Convey the purposes and services of a program to the user population and the impact that program or service would have.
