Key Responsibilities Conduct outreach to individuals who may need social and healthcare support. Build trusting relationships with patients and families. Complete Health-Related Social Needs (HRSN) screenings. Connect individuals with community resources and healthcare services. Coordinate referrals and follow up to ensure services are received. Work collaboratively with care coordinators, healthcare providers, and community partners. Document interactions and referrals in electronic systems. Participate in team meetings, trainings, and supervision sessions. Support patients in setting and achieving personal health and wellness goals. Assist with community outreach and engagement activities. What We're Looking ForRequired Qualifications High School Diploma or

GED. 1-3

years of experience in human services, healthcare, social services, community outreach, or a related field. Excellent communication and interpersonal skills. Strong organizational and time management abilities. Ability to work effectively with people from diverse backgrounds. Basic computer proficiency, including Microsoft Office applications. Valid New York State driver's license, reliable transportation, and ability to travel locally when needed. Preferred Qualifications Associate's degree in Human Services, Social Work, Public Health, or a related field. Community Health Worker experience. Experience working with Medicaid populations or underserved communities. Bilingual skills are a plus. Equivalent combinations of education and experience will be considered.

This position is funded through New York State's Social Care Network (SCN) Demonstration Waiver and is expected to continue throughout the duration of the waiver program. Ongoing employment is contingent upon continued waiver funding and program renewal 3/31/2027.
