$1200 sign on bonus may apply Unity House of Troy is seeking a compassionate and motivated Social Determinants of Health (SDOH) Liaison to join our Supported Housing Program serving individuals living with

HIV/AIDS.

This position focuses on addressing the social and environmental factors that impact health by connecting participants to housing, healthcare, financial assistance, and other essential community resources. The SDOH Liaison works collaboratively with healthcare providers, community agencies, and participants to improve access to care, promote housing stability, and support long-term wellness. This position may offer a hybrid work schedule while requiring regular on-site availability throughout the Capital Region area. Key Responsibilities Connect participants to supportive services that improve health outcomes and overall well-being. Assist participants in accessing emergency financial assistance, transitional housing, healthcare, and other community resources. Complete comprehensive assessments, individualized support plans, and all required program documentation. Improve access to care through referrals, care coordination, and ongoing follow-up. Serve as a liaison between participants, healthcare providers, and community service organizations. Build collaborative relationships with medical providers, housing agencies, and community partners. Collect, maintain, enter, and track participant data to ensure compliance with funding and reporting requirements. Maintain accurate, timely documentation within electronic databases and agency systems. Practice a harm reduction and participant-centered approach to service delivery. Promote a culture of inclusion, belonging, dignity, and respect. Perform other duties as assigned. Qualifications Associate's or Bachelor's degree in Health, Human Services, Education, Social Work, or a related field preferred; equivalent experience may be substituted for education. High School diploma or GED required. Minimum of one (1) year of experience working with individuals living with HIV/AIDS or other at-risk populations. Prior case management experience is required Experience working with adults living with serious mental illness and/or substance use disorders is highly desirable. Strong knowledge of community resources, care coordination, and case management principles preferred. Excellent communication, interpersonal, organizational, and problem-solving skills. Experience with electronic documentation, data entry, and Microsoft Office applications. Valid New York State Driver License, reliable transportation, and an acceptable driving record required.
