Community Health Worker (CHW)

Description :

The Community Health Worker (CHW) is responsible for helping clients access community services, other resources, and adopt healthy behaviors. The CHW assists clients in overcoming barriers to health, social services, education, and employment. The CHW also provides social support, informal counseling, and advocates for individuals and community health needs.

Supervision :

Clinical Director/CHW Supervisor Qualifications :

High School Diploma or GED/Prior CHW Experience preferred

• Willingness to obtain the Community Health Worker Certification through the Ohio Board of Nursing
• Must successfully pass a background check and drug screen
• Reliable transportation
• Ability and willingness to provide emotional support, encouragement and motivation to clients
• Ability to work in diverse communities and among different races, cultures and classes
• Basic computer skills

Necessary Requirements :

Must possess strong organizational skills, be detailed oriented and have excellent verbal/written communication skills. Must be able to work effectively with agency employees, outside agencies and a diverse clientele

$14.00
