Job Description
Job Summary St. Christopher's Hospital for Children The Community Health Worker will work primarily with specific target populations including but not limited to non-English speaking populations of geographic target areas. The Community Health Worker will work closely with internal and external stakeholders or partners including but not limited to clinical/medical/dental providers, primary care teams, and other agencies to improve patient care and outcomes. Patient outreach interactions may be telephonic, video/digital, or in-person or offsite. Depending on department, Community Health Worker may need to be bilingual or be knowledgeable of specific department terminology such as medical or dental. Screen for health referrals outside of hospital Commute to patient homes for follow-up visits; Reliable transportation required Knowledge of behavioral health
