Now Hiring!!! HFA Bilingual Advocate Bucks, Lehigh, Montgomery, and Philadelphia counties Join MCC as a Case Manager (HFA Advocate) - Make a Lasting Impact, One Family at a Time! Are you?... A passionate, community-driven advocate ready to support families through strength-based, trauma-informed home visiting services? Do you thrive on flexibility, building meaningful relationships, and helping parents create brighter futures for their children? Maternity Care Coalition (MCC) is hiring an HFA Advocate to provide high-touch, in-person support to pregnant people and parenting families throughout our Pennsylvania service area—including Bucks, Delaware, Lehigh, Montgomery, and Philadelphia counties.
What You'll Do:
Provide intensive, weekly in-home visits to families navigating complex challenges like trauma, substance use, mental health concerns, and child welfare involvement. Empower families using evidence-informed parenting, health education, and child development strategies. Develop individualized Family Service Plans and connect families to vital resources—from health care to housing. Promote early childhood development and reproductive wellness through community outreach and parenting support. Document all interactions accurately and maintain records using MCC's case management systems.
This Role Is For You If You:
Love being on the move. You'll regularly travel across multiple counties and work face-to-face with families in their homes and communities. Embrace flexibility. This role requires non-traditional hours, including evenings and weekends, based on the based on the needs of the families served. Are a relationship-builder. You value trust, inclusion, and reflective supervision to support lasting impact.
Must-Haves:
Associates, Bachelor's or Master's degree in Social Work, Public Health or related field. Valid driver's license and clean driving record. Experience with case management, community health, or supporting families with complex needs. Strong documentation and organizational skills. Comfortable working independently
