Community Health Navigator Summary of Duties:

Healthy Community Alliance is seeking a Community Health Navigator to provide person-centered navigation services that connect community members with resources and services to improve their health. This position is responsible for delivering comprehensive screening and navigation services to community members, including conducting thorough assessments of community member needs, identifying appropriate services and resources, assisting community members in accessing those services, and monitoring progress to ensure successful outcomes. Additionally, the Community Health Navigator will become a certified Health Insurance Navigator through the New York State of Health, the state's official Health Plan Marketplace, to help community members complete electronic applications and determine eligibility for health insurance coverage via the NY State of Health web portal.

Essential Duties and Responsibilities:

Provide empathetic and culturally competent screening for social determinants of health to community members telephonically, virtually, and/or in-person. Process daily screens and referral intake, including calls with potential program participants, documentation, and insurance/eligibility verification. Receive, prioritize, monitor, and triage incoming screenings and referrals, ensuring successful and timely connections between community members and the community-based organizations or partners that best meet their needs. Complete individual and family assessments to identify needs, strengths, goals, and barriers, and help address barriers that prevent access to healthcare and community resources. Help community members navigate, access, and connect with appropriate health, wellness, social service, community resources and providers. Assist community members with health insurance applications, eligibility, and enrollment through the NY State of Health web portal and provide education on health insurance options. Conduct follow-up with community members and maintain effective communication with community members and/or referring organizations to ensure referrals are successfully resolved or redirected as needed. Advocate for community members and provide support throughout the navigation process, handling questions and concerns with excellent customer service, empathy, and patience. Conduct and participate in community outreach events, health fairs, and educational activities. Build and maintain strong, positive working relationships with community partners, including social service agencies, healthcare providers, transportation providers, housing authorities, and other service organizations. Maintain accurate and timely community member documentation and data entry. Meet established monthly goals for applications, enrollment, outreach, and follow-up. Maintain confidentiality and comply with all organizational and program policies, including federal, state, and local HIPAA and Fraud, Waste, and Abuse regulations. Participate in required meetings, training, and professional development activities.

Education, Training, Experience and Certification:

High School Diploma/GED with 1 year of related experience. Bachelor's degree in social work, public health or human services encouraged. 1 year experience with health insurance enrollment (or other online benefit enrollment) desirable.

Requirements/Qualifications:

Experience in service coordination, health insurance enrollment, benefits enrollment, community outreach, case management, or assisting community members in improving healthy behaviors preferred, or Community Health Worker certification. Successful experience working in community, healthcare, public health, non-profit, or similar operations. Comfortable working with computers, online applications, web-based systems, and data entry, with sufficient writing ability to maintain accurate community member/family records, referral forms, and service coordination documentation. Strong organizational skills, attention to detail, and ability to manage time, schedules, and a community member caseload effectively while multitasking in a fast-paced, dynamic environment. Ability to adapt quickly to changing priorities and respond effectively to evolving job demands. Proven ability to meet or exceed performance metrics and objectives on a regular basis. Excellent interpersonal, communication, and customer service skills, with a friendly, compassionate, and empathetic personality and a proven ability to build rapport quickly. Capacity to inspire and motivate others. Ability to work independently and as part of a team. Reading ability is sufficient to comprehend training materials and assist community members with completing forms. Ability to work respectfully and effectively with individuals and families from diverse backgrounds. Travel locally throughout Chautauqua and Cattaraugus Counties as needed.
