Job Overview We are seeking a dynamic and dedicated Registered Dietitian Certified Diabetic Educator (RDN with CDCES credential) to join our healthcare team in a combined clinical and educational role. This position offers an exciting opportunity to empower patients managing diabetes and other chronic conditions through personalized nutrition therapy, comprehensive care plans, and expert education. As a vital member of our multidisciplinary team, you will deliver evidence-based medical nutrition therapy (MNT), conduct patient assessments, and collaborate closely with healthcare professionals to optimize health outcomes. Your expertise will help improve quality of life for diverse populations, including geriatrics, long-term care residents, and individuals requiring specialized dietary support. This paid position emphasizes professional growth, meaningful patient interaction, and impactful health coaching. Responsibilities Provide individualized nutrition counseling and medical nutrition therapy (MNT) to patients with diabetes, chronic diseases, or specific dietary needs Develop, implement, and monitor care plans tailored to each patient's health status, lifestyle, and nutritional requirements Conduct comprehensive patient assessments including diagnostic evaluation, weight management, tube feeding needs, and nutritional analysis Educate patients on blood glucose management, carbohydrate counting, insulin use, weight loss strategies, and lifestyle modifications using motivational interviewing techniques Collaborate effectively with physicians, nurses, endocrinologists, and other healthcare professionals to coordinate care plans and optimize treatment outcomes Manage electronic health records (EHR/EHR systems), documenting all patient interactions accurately and efficiently Supervise or mentor junior staff or interns in clinical nutrition practices and documentation standards Qualifications Registered Dietitian Nutritionist (RDN) credential with active licensure or certification as a Certified Diabetes Care and Education Specialist (CDCES) Proven experience in clinical nutrition settings such as hospitals or long-term care facilities with acute care experience preferred Strong knowledge of physiology related to metabolism, diabetes management, weight loss programs, and nutrition support including tube feeding Skilled in health coaching techniques such as motivational interviewing to promote behavior change Familiarity with EMR/EHR systems for documentation and patient management Experience working with diverse populations including geriatrics and individuals with complex dietary needs Excellent client communication skills coupled with the ability to collaborate seamlessly within multidisciplinary teams This role is designed for passionate professionals eager to make a tangible difference in patients' lives through expert nutritional guidance. $25.00 - $40.61 per hour
