JOB SUMMARY
As a Registered Dietitian with our PACE program, you'll work closely with an interdisciplinary team to support the health and well-being of older adults. You'll provide nutrition assessments, counseling, care planning, and oversee food services while building lasting relationships with the participants you serve. This is a full-time, Monday-Friday position with a consistent daytime schedule.
DUTIES & RESPONSIBILITIES
Clinical Care Complete nutrition assessments. Develop individualized nutrition care plans. Provide nutrition counseling and education. Monitor participant nutritional status and make recommendations to the interdisciplinary team. Interdisciplinary Care Collaborate with physicians, nurses, therapists, social workers, and other interdisciplinary team members. Participate in comprehensive care planning to support participant health and independence. Food Service Oversight Oversee meal quality and ensure adherence to therapeutic diets. Monitor kitchen operations, food safety, and inventory. Provide guidance and training to dietary staff.
EDUCATION/EXPERIENCE
Registered Dietitian (RD/RDN) eligible to practice in Colorado. 2-3 years relevant experience working as an RD. ServSafe certification or complete within 60 days of hire. Must possess BLS/CPR certification or complete within 60 days of hire.
