What You'll Do:

Nutrition assessments and patient care planning. Educate patients regarding nutrition to promote wellness. Monitor weight, nutritional status, wounds, and nutrition support needs, including tube-feeding management when applicable. Collaborate with RDs, Physicians, Speech & Occupational Therapists. Help improve patient outcomes using "Food as Medicine".
