Job Description

Phoenix Center is seeking a Registered Dietitian to join the clinical team. This role is responsible for assessing nutritional needs, developing care plans, and supporting residents and patients through individualized nutrition services. The ideal candidate has experience within long-term care and/or pediatrics and is comfortable working collaboratively within an interdisciplinary healthcare environment. Key Responsibilities Conduct nutritional assessments and develop individualized care plans Monitor resident and patient nutritional status and adjust interventions as needed Collaborate with Nursing, Dietary, and clinical leadership teams Ensure compliance with regulatory and dietary standards Participate in care planning and interdisciplinary meetings Educate residents, families, and staff on nutrition-related needs Maintain accurate and timely documentation Qualifications Active Registered Dietitian license/certification required Long-term care and/or pediatrics experience required Strong knowledge of clinical nutrition and therapeutic diets Strong communication and organizational skills Ability to work collaboratively within a team environment What's Being Offered Competitive compensation aligned with experience Supportive clinical environment Opportunity for long-term growth within the organization IND789
