This is a hybrid position with some hours completed in-office and some remotely from home, providing support in our outpatient clinics as well as to children and families in our Birth to Three program. The ideal candidate will possess a strong foundation in clinical nutrition, physiology, and dietetics, with a passion for improving patient health through personalized nutritional care. This role offers an opportunity to work collaboratively with a wonderful team of Occupational Therapists, Physical Therapists, Speech-Language Pathologists, and Clinical Social Workers. Our mission is to provide access to comprehensive, evidence-based, high quality care to all clients and families in a warm and respectful environment, where clients can meet their unique and individualized goals to improve their overall quality of life. Responsibilities Conduct thorough patient assessments to evaluate nutritional status and identify specific dietary needs. Create personalized, practical, and evidence-based nutrition plans for both pediatric and adult populations while tailoring information to staff and families with varying educational, verbal comprehension, and developmental needs. Initiate activities of daily living and other family-based routines involved with meal times. Provide nutrition counseling and education to clients, utilizing motivational interviewing techniques to foster behavior change. Collaborate with healthcare professionals such as referring physicians, and our team of Occupational Therapists, Physical Therapists, Speech Therapists, and Clinical Social Workers to coordinate patient care strategies. Manage medical nutrition therapy (MNT) for patients with chronic illnesses, including diabetes, cardiovascular disease, and gastrointestinal disorders. Oversee the implementation of Nutrition support services such as specialized diets and management of medically complex conditions. Utilize electronic health records (EHR/EHR systems) for documentation, progress tracking, and diagnostic evaluation. Stay current on advancements in clinical nutrition, physiology knowledge, and best practices for dietary management. Qualifications Licensure in the state of Connecticut as a Registered Dietician Nutritionist (RDN) or a Certified Dietician Nutritionist (CDN) Extensive knowledge of physiology, medical nutrition therapy (MNT), and dietary management for various health conditions. Strong skills in nutrition counseling, client communication, and motivational interviewing techniques. Experience with EMR/EHR systems for documentation and patient record management. Familiarity with tube feeding protocols, care plans, diagnostic evaluation, and nutritional analysis tools. Ability to work collaboratively with multidisciplinary healthcare teams to optimize patient outcomes. Experience working with individuals who have specific dietary needs such as pediatrics, patients with chronic diseases and/or feeding difficulties is highly desirable.
