Full-time, exempt Position Summary The Director of Nursing leads all nursing services at Spire Surgical Center, an ambulatory surgery center specializing in interventional pain management and spine procedures. The DON is responsible for safe, high-quality perioperative care from the pre-op call through discharge. The role supervises clinical staff, owns nursing policies and competencies, and keeps the center survey-ready for AAAHC accreditation and Medicare (CMS) Conditions for Coverage. Essential Duties and Responsibilities Clinical leadership and patient safety Oversee all pre-op, intra-op and PACU nursing care for local, MAC, sedation and general anesthesia cases. Review every pre-op call chart across all tiers and sign off to close it. The DON is the sole sign-off on the pre-op form. Escalate clinical concerns (anticoagulant holds, cardiac history, uncontrolled diabetes or hypertension, exclusion criteria) to both the proceduralist and the Director of Anesthesiology. Enforce the Pre-Op Call Patient Safety Optimization SOP, including medication holds, NPO, escort and transportation rules, and day-of screening. Act as a clinical resource on the floor and fill in for clinical staff as needed. Lead emergency preparedness, including malignant hyperthermia, code and transfer drills, and the crash cart and emergency equipment checks. Staff management Hire, orient, schedule and supervise RNs, LVNs, surgical techs and medical assistants, including W-2 employees and 1099 contractors. Verify licenses and certifications at hire and before they expire. Develop and maintain annual competencies and skills checklists, including sedation monitoring, PRP handling, sterile technique and emergency response. Conduct performance evaluations and address performance or conduct issues. Approve time records for nursing staff according to the payroll cutoff schedule. Regulatory, accreditation and quality Keep the center survey-ready for AAAHC accreditation (Medicare deemed-status track), CMS Conditions for Coverage, California Department of Public Health (CDPH) and Cal/OSHA. Own nursing policies and procedures, and review them at least annually. Lead the infection prevention program, including hand hygiene audits, sterilization and high-level disinfection oversight, and surveillance for surgical site infections. Participate in the Quality Improvement / Quality Assurance (QI/QAPI) committee. Track and report on adverse events, transfers, cancellations and near-misses. Oversee medication management and controlled-substance handling, including counts, discrepancy resolution and waste documentation. Maintain required logs and keep them accurate: PRP/implant lot logs, sterilizer, refrigerator and temperature logs, and crash cart checks. Participate in credentialing and privileging for nursing and allied health staff. Operations Coordinate with scheduling so cases are booked only after pre-op clearance. Manage clinical supply par levels and equipment maintenance, and work with vendors on clinical products. Handle patient complaints and grievances related to nursing care. Support the staff injury and exposure protocol, including sharps injuries and DWC-1 forms. Qualifications Required Active, unrestricted California RN license Current BLS and ACLS At least 5 years of perioperative, PACU or ambulatory surgery nursing experience At least 2 years in a charge, supervisory or management role Working knowledge of moderate sedation, MAC and general anesthesia recovery standards Familiarity with AAAHC or Joint Commission standards and CMS ASC Conditions for Coverage Preferred BSN or MSN CNOR, CAPA or CPAN certification PALS certification Experience in interventional pain or spine procedures, including SCS, RFA and ESI Direct experience preparing for or leading an accreditation survey Bilingual English/Spanish Skills Strong clinical judgment and willingness to escalate Clear communication with physicians, staff and patients Organized and detail-oriented with documentation and logs Comfortable with EHR and scheduling systems and with Google Workspace Physical Requirements Able to stand and walk for extended periods, lift or move up to 50 lbs with assistance, and wear PPE. May be exposed to bloodborne pathogens, radiation (fluoroscopy) and chemical disinfectants.
