Monitors staff schedules and the quality of nursing care at the facility. Maintains professional license or certification requirements.
Wellness Director We are seeking an experienced and compassionate Wellness Director to lead resident care and clinical operations in a residential care setting serving individuals with Serious Mental Illness (SMI) . This is an excellent opportunity for a healthcare leader who is passionate about quality care, team development, regulatory compliance, and helping residents maintain their health, safety, independence, and overall well-being.
What You'll Do:
Oversee daily resident care and clinical services Conduct pre-admission assessments and ongoing resident evaluations Coordinate physician orders, medications, pharmacy services, and outside healthcare providers Monitor changes in resident condition, incidents, falls, and hospitalizations Lead, schedule, train, supervise, coach, and evaluate care and medication staff Ensure appropriate staffing based on resident needs and regulatory requirements Oversee medication management, audits, documentation, and compliance Maintain accurate resident and staff records Ensure compliance with Title 22 and Community Care Licensing regulations Investigate and document incidents, complaints, injuries, and unusual occurrences Maintain safety and infection-control standards Participate in care conferences, emergency preparedness, safety initiatives, and quality improvement Serve as a primary clinical contact for residents, families, and responsible parties
What We're Looking For:
RN or LVN required Bachelor's degree in Nursing, Healthcare Administration, Gerontology, or a related field preferred 2+ years of supervisory experience in a related healthcare setting Current First Aid and/or CPR certification Experience or comfort working with elderly, dementia, and/or disabled individuals Knowledge of Resident Rights and mandated reporter responsibilities Familiarity with electronic health records and Microsoft Office Strong leadership, communication, organization, and decision-making skills Ability to work in a 24/7 residential care environment and support staffing needs as required We believe strong teams are built through Continuous Improvement, Humility, Ownership, Integrity & Transparency, Collaboration, and Excellence.
