Full Time Responsible for performing day to day implementation and follow up case management for patients in Cardiac Rehab. Provides individualized rehab program to meet personal and family needs of patients. Position is responsible for outpatient classes. Position requires understanding of cardiovascular disease. Responsibilities Assesses patients physical, psychosocial, educational needs on admission. Sets realistic goals. Individualizes patients plan of care. Performs no invasive cardiology tests. Documents patient care assessments.
