Job Summary We are seeking a dedicated and experienced Qualified Professional to join our team in leading and coordinating Intensive In-Home (IIH) services for children and adolescents at risk of out-of-home placement. IIH is a collaborative, team-based intervention designed to support children and adolescents (up to age 20) who experience serious and chronic emotional, behavioral, or substance use disorders. These individuals require intensive, community-based interventions to maintain stability and avoid out-of-home placements. The Qualified Professional will deliver in-person services primarily within the client's living environment, including homes and other community settings. The QP will collaborate closely with clients, families, caregivers, and multidisciplinary team members under the direct supervision of the IIH Team Leader. Evening visits may be required to accommodate school and work schedules. DUTIES:

Provide care under the supervision of a Team Leader, that includes direct and indirect interventions. Provide structured, face-to-face services to offer support and guidance across all functional domains of client care. Work in partnership with caregivers to support the implementation of effective, home-based behavioral support strategies. Provide crisis management support to assist individuals and families in de-escalating current crisis situations and reducing the likelihood of future occurrences. Deliver psychoeducation to individuals and their families regarding the diagnosis, condition, and available treatment options. Provide parenting skills training to support families in developing effective strategies for coping with their child or adolescent's behavioral or emotional disorder. Utilize motivational interviewing techniques to engage clients and support them in identifying and achieving their personal goals. Administer standardized assessments under Team Leader supervision. Provide case management services to ensure continuity of care through comprehensive assessment, service planning, linkage to resources, and appropriate referrals. Coordinate and arrange psychological and psychiatric evaluations as needed to support comprehensive client care. Provide and coordinate behavioral health services and interventions for individuals and their family members in collaboration with other licensed professionals. Monitor and document the individual's progress and evaluate the effectiveness of strategies and interventions outlined in the PCP. Attend treatment team meetings and other relevant meetings as required to ensure coordinated care and communication. Demonstrate willingness to accept supervision and actively engage in ongoing training to enhance knowledge and skills, ensuring current competency in job responsibilities, therapeutic interventions, and treatment modalities. Maintain documentation in compliance with agency policies and Clinical Coverage Policy requirements.

QUALIFICATIONS

•Bachelor's degree in a human services field with 2+ years of full-time, post-bachelor's degree

MH/DD/SA

experience OR•Master's degree in a human services field with 1+ year of post-graduate experience•A minimum 1-year experience working with children and adolescents who have mental, behavioral, emotional, or substance use disorders.
