Job Description

Youth Extensions is currently hiring a Qualified Professional for our Intensive In-Home program serving consumers aged 5-18. The QP works closely with youth, caregivers, and the treatment team to build skills, address behavioral and emotional needs, and help families achieve meaningful goals and greater stability. This is a contracted 1099 position - flexible hours (can include late afternoon/early evening)
Compensation:
$23-$25 per billable hour
Requirements:
  • Bachelor's degree in a human service field AND 2 years full-time post-bachelors experience in mental health or human services
  • Driver's license and reliable personal transportation
Responsibilities:
  • Implement and support home-based behavioral plans in collaboration with youth and caregivers
  • Review and update the Person-Centered Plan (PCP), ensuring its effective implementation
  • Monitor recipient progress and assess the effectiveness of strategies and interventions in the PCP
  • Link individuals to clinically appropriate services and alternative resources as needed based on CFT recommendations
  • Provide therapeutic interventions for individuals and families, including substance use treatment and coping skills development
  • Evaluate and intervene in ongoing crises to reduce the risk of recurrence
