• $125,000 per year Job Details
• Provide leadership and oversight for the Case Management department, including 11 direct reports (RNs, LPN, MSWs, LSW, and other allied health professionals) Guide safe, timely, and effective discharge planning across the hospital Support Utilization Review processes in coordination with the case management team Monitor and manage case manager caseloads (typical caseload of 13 patients per CM); step in to carry a patient assignment when census is low or staffing is short Maintain working knowledge of case managers' patient assignments and actively support the team as needed Foster strong interdisciplinary communication across nursing, therapy, and hospital leadership Partner with the RDCM on training, orientation, and ongoing education initiatives Participate in hospital leadership meetings and contribute to department strategy A bit about us:•A well-established Inpatient Rehabilitation Facility (IRF) serving the Yuma, AZ region, currently undergoing a 20-bed private-room expansion (bringing total capacity to 81 beds, opening 2027).
