Overview
DUTIES AND RESPONSIBILITIES CLINICAL DIRECTOR
- Provide clinical oversight and nursing supervision for individuals with Intellectual and Developmental Disabilities (IDD) receiving services under Georgia Medicaid Waiver programs (NOW, COMP and other Waivers)
- Conduct face-to-face initial nursing assessments upon authorization of services to establish baseline health status.
- Reviews monthly nursing assessments or documented monthly nursing notes to monitor health status and identify deviations from baseline.
- Perform timely incident follow-up assessments for emergency room visits, hospitalizations, or changes in condition.
- Review medical history, physician orders, diagnoses, medications, laboratory results, treatment protocols, and health data tracking logs for CLS and Skilled Nursing individuals.
- Communicate assessment findings and coordinate care adjustments with physicians, physician assistants, or nurse practitioners.
- Develop, implement, and revise individualized Healthcare Plans (HCP) and/or Risk Mitigation plans based on nursing assessments and physician orders.
- Review and update Care Plans every 62, 92, or 122 days as required.
- Develop and revise the client Service Plan of Care in coordination with the Individualized Service Plan (ISP).
- Review Service Plans during each site of the RN or LPN to ensure compliance with DBHDD policies, Georgia Medicaid regulations, CARF and internal quality standards.
- Review and update Health Risk Screening Tool (HRST) assessments for individuals scoring Level 3 and above and complete required clinical reviews.
- Facilitate referrals to appropriate clinical professionals based on HRST findings and service considerations.
- Conduct unannounced supervisory home visits to evaluate implementation of care plans and monitor overall health and safety for the individual.
- Review clinical record reviews, nursing documentation reviews, and supervisory visit documentation.
- Assist at least annually with supervisory visits to observe and evaluate PCA performance.
- Verify accuracy of physician orders, pharmacy transcriptions, and Medication Administration Records (MAR).
- Observe and monitor medication administration at least monthly to ensure compliance and accuracy.
- Identify and address medication discrepancies, omissions, and adverse reactions and train staff promptly on corrective actions.
- Monitor PRN medication usage, side effects, and treatment effectiveness, and report concerns to prescribing providers.
- Provide training and oversight to Direct Support Professionals (DSPs) regarding implementation of health-related interventions.
- Train and monitor proxy caregivers in accordance with Georgia Nurse Practice Act Rule 111-8-100 for health maintenance activities.
- Review health data tracking logs including vital signs, bowel movements, seizure logs, intake/output, skin integrity, repositioning, and sleep monitoring at least monthly.
- Report incidents in accordance with DBHDD Policy 04-106 (Reporting Deaths and Other Incidents in Community Services).
- Ensure individualized supports such as special diets, medical equipment, and specialized supplies are implemented per ISP, HCP, and Plan of Care.
- Participate in development and implementation of infection control procedures and communicable disease prevention measures.
- Serve as liaison between individuals, families, guardians, caregivers, and healthcare providers to promote optimal health outcomes.
- Ensure all RN documentation complies with DBHDD requirements, Comprehensive Supports and NOW Waiver Program standards, GAMMIS guidelines, CARF and generally accepted nursing standards.
- Review RN, LPN and DSP notes for specific tasks performed, date of service, start and end time, service location, progress toward goals, and proper signature with credentials.
- Perform additional duties within the scope of nursing practice as assigned by the
CEO. MININUM QUALIFICATIONS
- Graduation from an accredited Bachelor of Nursing Program (BSN).
- Active Registered Nurse License in the State of Georgia.
- Minimum of 3-5 years of nursing experience in HRST and IDD waiver programs, preferably
NOW/COMP.
- Strong interpersonal skills with individuals, families, caregivers, and healthcare providers.
- Demonstrated ability to supervise and train RNs, LPNs, and DSPs.
- Proficient in EVV systems, Microsoft Office Suites, IMAGE, IDD Connect and HRST.
- BLS/CPR Certification
- Additional continuing education in IDD, behavioral health, and skilled nursing.
- Must meet all pre-employment requirements, including a TB screening and criminal background check.
Pay:
$80,000.00 - $110,000.00 per year
