FUNCTIONS AND DUTIES OF THE JOB

(This list is not exhaustive and may be supplemented as necessary.) Admission of patients. Processes inquiries/referrals for admission by determining the appropriateness of services consistent with agency policy and procedures. Ø Determine the patient's and family's initial needs through patient and family assessment, consultation with the attending physician, and collaboration with other interdisciplinary team members. Ø Admits patients, obtains physician orders, writes, develops, and administers an initial plan of care, and completes related documentation per agency policy and procedures. Ø Instructs on administering medications, disease process, symptom control measures and other treatments as prescribed by the patient's physician and care plan. Ø Apply an appropriate cost containment policy when initiating and overseeing the patient care plan. Ø Facilitates a timely transition of patient and family care to the primary nurse. Ø Participates in Interdisciplinary Group and nursing meetings to direct nursing plans for patients. Ø Ensure compliance with HIPAA and the protection of health information. Develop Referral Sources Develop and maintain relationships with referral sources Follow up to ensure that Family Hospice is considered when referrals are made. Interact with Family Hospice staff, encouraging them to assist with relationship-building in the community and identify referral sources. Assist with the development and presentation of educational programs, both formal and informal. o Assist with educational training for outside third parties. Perform other needed and related duties as assigned.
