A regional hospice provider in Chicago's western suburbs is looking for an experienced hospice nurse leader to serve as Director of Clinical Services. This role is for someone who wants to lead a clinical team, build strong interdisciplinary care, and make sure every patient and family receives compassionate, high-quality end-of-life care. In this role, you will lead the clinical operations of the hospice program, including: Overseeing all direct and indirect patient care services delivered by the hospice clinical team, including RNs, LPNs, and hospice aides Leading and coordinating the interdisciplinary group (IDG) process, making sure plans of care are developed, reviewed, and updated on time and reflect each patient's and family's goals Partnering with the Medical Director on admissions, hospice eligibility, certifications, and recertifications Reviewing referrals and intake information to evaluate hospice appropriateness before assigning clinical staff Overseeing pain and symptom management practices and making sure the team delivers the appropriate level of care, including routine home care, general inpatient, respite, and continuous care Collaborating closely with social work, spiritual care, bereavement, and volunteer services to support the whole patient and family Ensuring 24/7 clinical coverage, including oversight of on-call processes Maintaining complete, accurate clinical records and ensuring compliance with the Medicare Hospice Conditions of Participation, CHAP standards, and state and federal regulations Performing chart audits and leading or participating in QAPI activities, survey readiness, and quality reporting, including HOPE and Hospice CAHPS Supervising, coaching, and developing clinical staff, including probationary and annual performance evaluations Leading orientation for new clinical team members and ongoing in-services for current staff Making patient visits in the home or facility setting as needed Ensuring medical supplies are adequate and appropriately managed Building positive relationships with patients, families, referral sources, and community partners To be considered, you will need: Graduation from an accredited school of nursing A current, unrestricted Registered Nurse license in Illinois At least 2 years of hospice nursing experience At least 1 year of supervisory or clinical leadership experience, preferably in hospice Working knowledge of the Medicare Hospice Conditions of Participation and CHAP standards Strong verbal and written communication skills A confident, positive leadership style, with the ability to keep a team engaged and focused on quality outcomes The ability to handle patients, families, and community partners with tact and professionalism Willingness to travel locally as needed This organization offers: A competitive benefits package, including medical insurance, life insurance, long-term disability coverage, and a 401(k) with company match after 1 year of service Access to earned wages before payday Extensive training, ongoing professional development, and a strong commitment to promoting from within We are an Equal Opportunity Employer.
