Job Description
A healthcare facility is seeking a Director of Case Management (RN) to provide leadership and strategic oversight for hospital case management operations. This on-site leadership role supports effective care coordination, utilization management, and discharge planning while collaborating with clinical and administrative teams to improve patient outcomes and continuity of care. Responsibilities Provide leadership and direction for hospital case management operations. Oversee care coordination, utilization review, and discharge planning functions. Develop and implement departmental strategies to support quality patient care and operational goals. Lead, mentor, and support case management staff while promoting professional development and accountability. Collaborate with physicians, nursing leaders, and administrative teams to improve care transitions and patient outcomes. Ensure compliance with regulatory standards, organizational policies, and best practices. Monitor department performance and identify opportunities for process improvement. Support effective communication among interdisciplinary teams to promote coordinated patient care. Requirements Current (or eligible) Montana RN license required. Minimum of one year of hospital case management leadership experience. Bachelor's degree required; advanced degree (MSN, Healthcare Administration, or MBA) strongly preferred.
