Full-Time Case Management Supervisor We are seeking an experienced and compassionate Case Management Supervisor to provide leadership and oversight to a team supporting individuals transitioning back into the community. Key Responsibilities Supervise and provide guidance to 3-4 Case Managers Oversee day-to-day case management and housing placement services Ensure participants have appropriate housing and service plans Coordinate care with internal and external service providers Facilitate team meetings and participate in case conferences Assist with crisis intervention and complex participant needs Support staff recruitment, onboarding, training, and performance evaluations Monitor documentation, client charts, quality assurance, and program compliance Conduct quality assurance reviews and support quality improvement initiatives Collaborate with program leadership to promote positive participant outcomes Ensure participants have access to services addressing employment, education, healthcare, housing, and other social determinants of health Maintain a caseload of high-need clients when necessary Qualifications Minimum of 3 years of experience providing case management or direct client services Previous supervisory or team leadership experience preferred Bachelor's degree in Social Services, Psychology, Social Work, or a related field preferred Master's degree with an active NYS LMSW, LCSW, or LMHC license preferred Strong leadership, communication, and organizational skills Experience working with individuals who may have complex medical, behavioral health, substance use, housing, or reentry needs Ability to effectively coordinate services across multidisciplinary teams Commitment to providing respectful, high-quality, participant-centered services Compensation & Benefits $75,000-$80,000 annually , commensurate with experience Comprehensive healthcare plan options Generous paid time off, with PTO beginning to accrue immediately Educational benefits that may be used toward eligible tuition costs, student loan reimbursement, and textbooks Opportunity to develop leadership and program administration experience About the Role The Case Management Supervisor will provide administrative and operational oversight of case management and housing placement services at a program located in Fresh Meadows, Queens .
