Job Summary We are seeking a dedicated and compassionate Care Coordinator/Patient Navigator to join our dynamic healthcare team. In this vital role, you will serve as a bridge between patients and the healthcare system, ensuring seamless access to services, comprehensive care coordination, and personalized support. Your energetic approach will help empower patients from diverse backgrounds, especially those from underserved communities or with complex health needs, to navigate their healthcare journeys confidently. You will work closely with medical providers, social workers, and community resources to facilitate effective discharge planning, behavioral health support, and ongoing patient engagement. Responsibilities Conduct patient intake assessments to gather comprehensive medical and social histories using electronic health records (EHR) systems. Coordinate care plans by collaborating with medical teams, social workers, behavioral health specialists, and community organizations. Provide patient education on health insurance options, Medicaid regulations, and healthcare services to promote informed decision-making. Assist patients with appointment scheduling, follow-up care, medication management, and discharge planning to ensure continuity of care. Maintain accurate medical records and documentation in compliance with HIPAA regulations and healthcare standards. Support patients in understanding medical terminology, CPT coding (Current Procedural Terminology), ICD-9/ICD-10 coding (International Classification of Diseases), and other billing processes. Work with individuals experiencing co-occurring disorders or mental health conditions by offering empathetic client communication and appropriate referrals. Utilize EMR/EHR platforms for data entry, case management tracking, and reporting to optimize patient service delivery. Ensure compliance with Medicaid regulations and health insurance policies while maintaining patient confidentiality at all times. Engage in culturally competent communication; bilingual skills are highly valued to serve diverse populations effectively. Experience Previous hospital experience or acute care setting exposure is highly preferred. Strong knowledge of medical coding practices including CPT coding and ICD coding (ICD-9/ICD-10). Experience working with individuals with mental health conditions or behavioral health needs is essential. Background in social work or case management enhances ability to coordinate complex care plans effectively. Familiarity with Medicaid regulations, healthcare support systems, and electronic health record management is required. Demonstrated ability to provide excellent customer service while maintaining patient confidentiality under HIPAA guidelines. Proven track record working with underserved communities or populations experiencing social determinants of health challenges is a plus.
