Job Description

Overview Vital Home Care is seeking a dependable and proactive Coordinator to join our growing team. The Coordinator is a central figure in ensuring that all client schedules are managed efficiently, caregiver attendance is monitored, and communication is seamless. This individual will work closely with clients, caregivers, and administrative staff. The ideal candidate will possess strong communication skills and be able to think outside of the box, and be a great listener. Responsibilities Respond promptly to all incoming calls; minimize missed calls and ensure professional communication. Manage daily schedules, update case assignments, and cover all open shifts.
Address call-outs immediately:
contact clients, confirm if replacements are needed, and coordinate coverage. Maximize visit utilization by filling open shifts and minimizing idle hours. Monitor the Call Dashboard in real-time, documenting all missed calls in accordance with agency policy. Notify clients and vendors of any changes in service promptly. Document all missed visits with clear reasons and communication. Record staff disciplinary issues on the Discipline Smart Sheet for further action. Record all incidents and status changes on the internal CRM Escalate all patient complaints to the Administrator immediately. Submit daily operational reports as required. Provide support to other departments in times of absence or workload overflow. Ensure all tasks and documentation are completed before end-of-day. Perform other duties as assigned.. Skills Excellent communication skills, both verbal and written, to effectively interact with patients, caregivers and team members. Previous experience in scheduling, staffing, or coordination preferred (especially in home care or healthcare settings). Proficient in Microsoft Office and basic computer applications. Excellent verbal and written communication skills. Strong attention to detail with the ability to multitask effectively in a fast-paced environment. Self-motivated with a strong sense of accountability and professionalism. Knowledge of agency policies and procedures (training provided upon hire). Why Join Us?
