The position provides focused outreach and scheduling support for online new patient requests, patient reengagement, preventive care, quality gap closure, and value-based care initiatives.
Community Need:
Our community continues to experience barriers to healthcare access, including difficulty scheduling appointments, reconnecting patients who have fallen out of care, and ensuring individuals receive recommended preventive and chronic disease services. These challenges can lead to delayed treatment, poorer health outcomes, and increased health disparities among vulnerable populations. Additional outreach and patient engagement support is needed to connect individuals to care, improve continuity of services, and increase access to preventive healthcare resources throughout the community.
Member Position Summary:
The Patient Access & Outreach Coordinator AmeriCorps member will help community members access healthcare services by conducting patient outreach, supporting appointment scheduling, reconnecting patients to care, and promoting preventive health services. Through these activities, the member will help improve patient engagement, reduce barriers to care, and support healthier outcomes for underserved populations. Member Placement:
A standard background check will be conducted. PPD, drug screening, and some vaccinations may be required, a representative of the host site will be in contact with the member should this be the case. Once onsite, the member will participate in organizational and departmental orientations and training during the first one-two weeks of placement.
Member Impact:
The member will gain valuable experience in patient outreach, healthcare navigation, customer service, and population health initiatives. Through direct service activities, the member will develop professional communication, problem-solving, and care coordination skills while learning how community health centers improve access to care and address health disparities. This experience will provide meaningful career development for individuals interested in healthcare, public health, and human services professions.
Essential Functions of Position:
1.
Patient Access and New Patient Coordination:
Manages and follows up on online new patient requests and other assigned access lists. Contacts prospective patients to identify service needs and facilitates connection to appropriate scheduling resources. Supports appointment scheduling and navigation of Trillium Health services. Maintains timely follow-up of patient inquiries and tracks scheduling outcomes. Escalates barriers, urgent needs, or unresolved requests to the appropriate supervisor or care team. 2. Patient Outreach and Re-engagement Conducts outreach to patients who are overdue for care, have not been seen within established timeframes, or are included in approved outreach campaigns. Uses telephone, MyChart, and other approved communication methods to support patient engagement. Assists patients with scheduling and provides information about available Trillium Health services. Documents outreach attempts, patient responses, barriers, and disposition in designated systems. Supports efforts to reduce outreach backlogs and improve continuity of care. 3. Quality and Population Health Support Conducts outreach to patients with identified quality care gaps. Supports scheduling for preventive screenings, annual wellness visits, and chronic disease followup appointments. Assists with targeted quality, population health, and value-based care outreach campaigns. Coordinates with Quality and clinical teams to ensure outreach follows approved workflows and scripting. Helps identify and communicate recurring barriers affecting patient access and engagement. 4. Data Tracking and Reporting Accurately records assigned outreach activity and outcomes. Maintains tracking logs, work queues, and reports as directed. 5.
Operational and Program Support:
Collaborates with Patient Service Representatives, Quality staff, Care Management, and clinical teams to coordinate outreach activities. Participates in department meetings, orientation, training, and process improvement activities. Completes duties within the approved scope of the AmeriCorps host-site placement and follows direction from the designated site supervisor. Performs other approved patient access and outreach projects as assigned.
Knowledge, Skills, and Abilities:
Customer service, patient services, healthcare, outreach, or community engagement experience preferred. Experience working with diverse populations and using electronic systems is preferred. Strong communication, customer service, organization, time management, documentation, and problem-solving skills. Ability to work independently and collaboratively, manage multiple priorities, maintain confidentiality, and communicate respectfully with diverse patient populations. Basic proficiency with Microsoft Office applications required

Required Academic and Experience Qualifications:
High school diploma or GED required. Associate degree or coursework in healthcare administration, public health, human services, social work, or a related field preferred. A complete package of benefits includes: Full-time CHCANYS AmeriCorps members accepted into this program will receive a living allowance of $26,500 (for 2026-2027 program year) plus health insurance benefits and, if eligible, childcare benefits. Please note that the living allowance is provided on a bi-weekly basis and comes to approximately $2,200 a month after taxes. After successfully completing a term of service, each member will receive an education award worth $7,395 to be used toward previously incurred student loans or future education costs. Payments are made directly to the educational institutions. Loan deferment on qualified student loans Members will benefit from ongoing professional development counseling and a wide exposure to careers in public health and medicine. Members receive extensive orientation and in-service training, including day-to-day training and supervision at the host sites.
