The Health Services Coordinator (RN) is responsible for the coordination and oversight of health and medical services for youth served by Safely Home, Inc. This position combines nursing oversight with medical case coordination to ensure that medical needs, medications, appointments, records, and follow-up care are managed accurately and timely. The Health Services Coordinator works closely with youth, families and guardians, healthcare providers, pharmacies, residential staff, clinical staff, and agency leadership to promote continuity of care and maintain compliance with physician orders, agency policies, professional nursing standards, and applicable regulations.ESSENTIAL DUTIES AND RESPONSIBILITIES
Health and Medical Coordination- Oversee the day-to-day medical and healthcare needs of youth.
- Monitor diagnoses, allergies, active medical conditions, treatment recommendations, and changes in health status.
- Coordinate medical, dental, vision, behavioral-health medication-management, specialty, and follow-up appointments.
- Schedule appointments, coordinate transportation, and accompany youth.
- Obtain and communicate provider orders, recommendations, prescriptions, and follow-up instructions.
- Track required appointments, referrals, laboratory work, and follow-up services.
- Obtain and document monthly vital signs and additional health monitoring as clinically indicated.
- Communicate with healthcare providers, pharmacies, hospitals, guardians, and agency staff regarding youth medical needs.
- Respond appropriately to illnesses, injuries, medication concerns, and urgent medical situations. Medication and MAR Oversight
- Maintain oversight of medication administration practices and Medication Administration Records (MARs).
- Ensure MARs accurately reflect current physician orders, medications, dosages, and administration instructions.
- Review and reconcile MARs monthly, with required review completed by the 5th of the following month.
- Monitor MARs for missing initials, omitted or refused doses, medication errors, documentation discrepancies, and other concerns requiring follow-up.
- Ensure medication changes, new orders, and discontinued medications are updated promptly.
- Coordinate prescription refills and pharmacy communication.
- Ensure medications are stored, secured, documented, and disposed of appropriately.
- Address medication errors, missed doses, adverse reactions, and related documentation in accordance with agency procedures.
- Provide guidance and education to staff responsible for medication administration.
- Administer medications as needed and within the scope of RN practice. Intake and Discharge
- Complete the medical portion of the admission process, including review of medical history, diagnoses, allergies, medications, physician orders, immunizations, and current healthcare needs.
- Identify missing medical information and obtain required records from guardians, placing agencies, pharmacies, or healthcare providers.
- Verify medications received at admission and establish or update the youth's MAR.
- Identify immediate healthcare needs and required follow-up appointments.
- Participate in discharge planning to ensure continuity of medical care.
- Coordinate medications, prescriptions, medical records, upcoming appointments, and follow-up recommendations at discharge.
- Ensure medical and medication documentation is accurate and complete through the youth's discharge. Medical Records and Compliance
- Maintain complete, accurate, and current medical records.
- Document medical appointments, provider recommendations, nursing interventions, medication changes, injuries, illnesses, and other relevant health information.
- Maintain current physician orders, MARs, medication records, laboratory results, immunization records, and appointment documentation.
- Review medical records routinely for incomplete documentation or required follow-up.
- Assist with medical documentation needed for audits, licensing reviews, accreditation, and quality-improvement activities.
- Maintain confidentiality of protected health information in accordance with HIPAA, agency policy, and applicable law. Collaboration and Other Responsibilities
- Participate as a member of the interdisciplinary treatment team.
- Communicate relevant medical information to clinical, residential, educational, case-management, and administrative staff.
- Notify agency leadership of significant medical concerns, hospitalizations, emergency-room visits, or medication issues.
- Participate in case reviews, treatment-team meetings, and discharge-planning meetings when medical input is needed.
- Provide health and medication education to youth and staff when appropriate.
- Maintain required professional certifications and participate in agency training.
- Perform other health-service duties consistent with the role and RN scope of practice as assigned.
QUALIFICATIONS
- Current, active, and unrestricted Registered Nurse (RN) license in the State of Ohio required.
- Nursing degree from an accredited nursing program required.
- Experience in pediatric, adolescent, behavioral health, psychiatric, residential treatment, community health, or similar settings preferred.
- Experience with medication management, MAR review, healthcare coordination, or medical case management preferred.
- Knowledge of medication administration, health assessment, documentation, infection control, and physician orders.
- Strong organizational, documentation, communication, and time-management skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
ADDITIONAL REQUIREMENTS
- Must be at least 21 years of age.
- Must successfully complete required BCI/FBI background checks and employment screenings.
- Must possess a valid driver's license and meet agency driving requirements.
- Must maintain current RN licensure and CPR/First Aid certification.
- Must complete required agency training and maintain applicable crisis-intervention certification.
- Must comply with agency confidentiality, infection-control, medication-management, and safety requirements.
PHYSICAL AND WORK ENVIRONMENT REQUIREMENTS
This is a full-time, on-site position within a youth residential treatment environment. The position requires movement throughout the facility and local travel for medical appointments, pharmacies, hospitals, laboratories, and other healthcare services as needed. The employee must be able to sit, stand, walk, bend, and perform routine nursing responsibilities throughout the workday and respond appropriately to urgent medical situations.EQUAL EMPLOYMENT OPPORTUNITY
