The ECM Program Manager will help manage and oversee day-to-day operations of both programs, including provisions of outreach, health and psychosocial education, linkage to care, patient advocacy, and programmatic oversite among others, under the direction of the Director of Cal

QUALIFICATIONS/LCENSURE
Bachelor's degree in nursing, medical, social work, sociology, psychology, public health, or health related field required. Supervision and Program Development, management and evaluation background One-year related experience working in a medical and/or social services Working knowledge of health information systems and data needed to monitor/assess utilization, performance management, and health outcomes. Experience preparing and delivering presentations. Knowledge of development of evaluation tools, and evaluation of health programs, data analysis, and report writing. Strong health care knowledge base required to strategically develop health program evaluations, manage health grants and work well with Managed Care Plans & other community organizations Knowledge of community health evaluation tools, approaches, frameworks, and methods. Grant budgeting knowledge. Bilingual English/Spanish preferred (read, write, speak)
RESPONSIBLITIES
Responsible for overall management and day-to-day tasks, support service provisions, quality measures, reporting and performance feedback for ECM program Assist in developing and growing ECM program, as needed to support eligible members Help manage all programmatic tasks as related to ECM program Help oversee day-to-day operations and support service provisions In-charge of hiring and training new team members for ECM program Help develop and engage in team and/or multi-disciplinary meetings Support Clinical Consultants, Lead Coordinators, Lead Care Managers, Community Health Workers, Housing Navigator and others as needed to help facilitate success of programs Work with all staff to support Care Plans and ensure that ECM members meet criteria for enrollment in services Help to provide mentorship and supervision for ECM staff under the guidance of Director of CalAIM Serve as an additional point of contact for patients and staff, and collaborating agencies Assist in coordinating all programmatic activities and tasks, including policy and procedures Assist in the coordination with St. John's providers and staff, as well as leadership and staff of partner agencies Maintain up-to-date knowledge and database of community resources and referral options for patients Ensure patient data is entered in E-Clinical Works, as required Work with Finance Manager to manage program budgets Submit ECM requisitions as required / needed Approve CONCUR reports for team Update Staff Capacity reports when needed & send to insurances (LA Care, Health Net, Anthem Blue Cross, Molina) Review and audit staff member's charts and documentation, as needed Help supervise, coordinate, and monitor program evaluation activities Help create and coordinate MOUs with all partner and referral agencies Write and edit progress reports for affiliated grants by coordinating with relevant staff to collect the necessary data to report on achievement and evaluation of contract goals and objectives Assist in creating ECM presentations, trainings, and technical assistance tools for St. John's providers and staff, under the guidance of CalAIM Director Assist in the development of education/promotional materials for ECM Attend meetings, trainings, and other work-related events as needed Work with CalAIM Director, Development Team and Chief Medical Officer to promote integration of ECM program Work collaboratively with other departments to identify and evaluate program opportunities according to overall strategic objectives Work with clinical team to integrate designated programs into clinical workflow Other duties may be assigned or may be modified as business needs dictate and/or client's needs dictate St. John's Community Health is an Equal Employment Opportunity Employer
