DESCRIPTION

The Department of Medicine is responsible for fulfilling the teaching, research, and clinical missions of the University of California, San Diego and employs 486+ full-time salaried faculty members and 100+ academic appointees. Staff personnel include 600+ staff in 9 bargaining units, students, and volunteers. There are 250 clinical faculty practicing at multiple sites. Under the general supervision of the social work supervisor, the Trans Health Project (THP) Case Manager provides comprehensive, trauma-informed, gender-affirming, and culturally responsive case management support services to Black and Latina transgender women, prioritizing participants enrolled in the THP initiative research project with UCSD. This position supports Black and Latina transgender women experiencing barriers to health care, HIV prevention and treatment services, housing stability, economic security, behavioral health services, and other social determinants of health. Using a participant-centered and strengths-based approach, the THP Case Manager assists participants in identifying goals, advocating for their needs across systems of care, accessing gender-affirming services, and addressing barriers to overall health and well-being. The position provides care coordination, resource linkage, and service advocacy, while working collaboratively with community partners and research collaborators to support participant engagement, retention, and connection to services, while ensuring confidentiality and compliance with organizational, funder, and research requirements. The ideal candidate possesses experience working with transgender and BIPOC communities, demonstrates cultural humility and responsiveness, and has knowledge of the unique challenges impacting Black and Latina transgender women. Spanish-language proficiency is strongly preferred.

MINIMUM QUALIFICATIONS

Possession of a bachelor's degree in social work from a program approved by the Council on Social Work Education, and one year of experience; or an equivalent combination of education and experience; and knowledges and abilities essential to the successful performance of the duties assigned to the position.

Computer/Office Equipment Skills:

Must be comfortable working in PC/Windows-based environments. Must be proficient with Microsoft Office. Demonstrated ability to manage multiple tasks with shifting priorities. Experience working with the LGBTQ community and persons living with or affected by HIV infection, particularly young Latina and Black transgender women, in San Diego County. Fluency in English. Must demonstrate competence in working with diverse populations. Knowledge of person-first language and inclusive language used in sexual, gender, and racial/ethnic minority populations.

PREFERRED QUALIFICATIONS

Fluency in Spanish. Must demonstrate competence in using strength-based, trauma-informed, and motivational interviewing techniques. Experience working with bilingual (English and Spanish) communities. Must have access to reliable transportation. Knowledge of HIV transmission, care, prevention, and pre-exposure prophylaxis (PrEP.

$63,078 - $83,019 (will be prorated if the appointment percentage is less than 100%)
