Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, mobility support, transfers, and supervision. Monitor patients' health by taking vital signs, observing for changes, giving medication reminders, and reporting findings when necessary. Assist with meal planning and preparation, making nutritious meals for clients. Provide transportation assistance for appointments, errands, or community activities to promote engagement and independence. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed. Document patient progress, update care plans, and communicate effectively with clients, families, and care teams to ensure comprehensive care delivery. Experience Prior experience working in a home care setting or other similar role is highly preferred. Demonstrated ability to observe clients closely for changes in condition or behavior. Knowledge of client monitoring techniques including vital signs measurement and patient transfers. Experience supporting individuals with developmental disabilities, neurodevelopmental disorders, behavioral issues, or autism spectrum disorder is a plus. Familiarity with HIPAA regulations, and emergency response protocols. Proven ability to handle heavy lifting safely and perform manual handling tasks effectively while maintaining employee and client safety. Strong communication skills to foster positive client interactions and collaborate effectively with client care teams.
