Responsibilities Assist residents with activities of daily living (ADLs), including personal hygiene, grooming, dressing, and mobility support using appropriate patient transfer methods such as Hoyer lifts and manual handling techniques. Monitor patient health by taking vital signs, observing for changes in condition, and reporting findings accurately while adhering to HIPAA privacy regulations. Support medication administration and ensure proper documentation in care plans, maintaining compliance with healthcare standards. Prepare nutritious meals and assist with meal planning to meet individual dietary needs, including specialized diets for dementia or Alzheimer's care. Provide companionship and emotional support while engaging residents in social activities and community outings to promote mental well-being. Maintain a clean and safe environment by performing cleaning duties, ensuring infection control protocols are followed, and managing home & community care areas effectively. Document all interactions, observations, and interventions clearly in resident records; communicate effectively with team members regarding resident progress or concerns. Skills Proven experience working with individuals with developmental disabilities, intellectual disabilities support, or senior care environments such as nursing homes or assisted living facilities. Strong knowledge of patient monitoring techniques including vital signs measurement, patient observation, and dementia or Alzheimer's care strategies. Familiarity with medication administration procedures, first aid, CPR certification, and HIPAA compliance to ensure safety and confidentiality. Ability to perform heavy lifting safely and assist with patient transfers using Hoyer lifts or other manual handling equipment. Excellent communication skills for engaging with clients, families, healthcare teams, and community resources effectively. Experience working with individuals with neurodevelopmental disorders or autism; adept at implementing personalized care plans aligned with residents' rights. Capable of assisting clients with personal transportation needs and supporting them during outings or appointments while maintaining safety standards.
