Job Description

Overview We are seeking a compassionate and dedicated Home Health Aide to provide essential support to individuals in their homes. The ideal candidate will have experience working with people with developmental disabilities and possess a strong commitment to enhancing the quality of life for our clients. This role involves assisting clients with daily living activities, ensuring their safety, and promoting their independence while providing emotional support. Responsibilities Assist clients with personal care tasks such as bathing, dressing, grooming, and changing of diapers. Support clients in daily living activities including meal preparation, medication reminders, and light housekeeping. Monitor and document vital signs and report any changes in the client's condition to healthcare professionals. Provide companionship and engage clients in meaningful activities to promote mental well-being. Assist clients with mobility and transfers, ensuring their safety during movement. Maintain a clean and safe environment for clients, adhering to health and safety protocols. Collaborate with family members and healthcare teams to develop personalized care plans. Help prepare meals. Skills Experience working with individuals with developmental disabilities or in long-term care settings is preferred. Strong knowledge of home health practices and patient observation techniques. Ability to monitor vital signs accurately and respond appropriately to changes in client health status. Compassionate approach towards patient care, with excellent interpersonal skills. Familiarity with memory care strategies and dementia care best practices is a plus. Strong organizational skills and attention to detail in documenting client progress. Ability to work independently while also being a team player within a healthcare setting.
