Summary We are seeking a Direct Support Professional to join our team at the Foundation of Exceptional Achievers, Inc. This role is vital in providing support and assistance to adults with developmental challenges, helping them achieve independence and integration into their communities. Responsibilities Assist individuals with daily living activities, ensuring their comfort and safety. Monitor patient health and report any changes or concerns to the appropriate personnel. Maintain a clean and organized living environment for residents. Provide long-term care and support tailored to each individual's needs. Assist with medication administration as required, following all safety protocols. Support residents in exercising their rights and promoting independence. Engage residents in meaningful activities that foster social interaction and personal growth. Perform heavy lifting when necessary to assist residents with mobility or transfers. Collaborate with a multidisciplinary team to develop and implement care plans. Qualifications Experience in home care, senior care, day program or long-term care settings is preferred. Implement plans of care; (i.e. Progress Notes, Outing Notes, Behavior Documents, etc.) Knowledge of patient monitoring techniques and resident rights is essential. Familiarity with working with individuals on the autism spectrum is a plus. Strong communication skills and the ability to work effectively as part of a team. Compassionate demeanor with a genuine interest in helping others. Ability to perform heavy lifting as needed while ensuring resident safety. Certification in CPR/First Aid is advantageous but not mandatory. Must be willing to undergo background checks as required by the organization.
