Home Health Aide & Caregiver

Job Description

This rewarding role involves providing essential support to clients in their homes or assisted living environments, promoting independence, safety, and comfort. Whether assisting with daily activities or offering specialized care, you'll be a vital part of a team committed to enhancing quality of life through attentive, respectful service. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and ensuring dignity and comfort. Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications safely following medical instructions and document medication administration in compliance with HIPAA regulations. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory-related conditions through tailored memory and dementia care techniques. Support patients with complex needs including catheter care, hospice care, and working with individuals with developmental disabilities or disabilities. Prepare nutritious meals and assist with light housekeeping tasks such as cleaning and laundry to maintain a safe and healthy environment. Document all patient interactions thoroughly in medical records while adhering to privacy standards; communicate effectively with healthcare teams regarding patient status. Skills Strong patient observation skills to detect changes in condition promptly and accurately. Experience working in assisted living facilities, nursing homes, or home health settings. Proficiency with EMR/EHR systems for medical documentation and record keeping. Knowledge of medical terminology, medication protocols, and procedures such as CPR certification. Ability to operate medical equipment including Hoyer lifts, vital sign monitors, and other assistive devices safely. Compassionate caregiver with experience supporting individuals with disabilities or chronic conditions like dementia or Alzheimer's disease. Excellent communication skills to coordinate care plans effectively while maintaining HIPAA compliance. Capable of performing light housekeeping duties including cleaning, meal preparation, and laundry to ensure a clean living space for clients.
