Job Summary We are seeking a compassionate and dedicated Certified Home Health Aide to provide exceptional in-home care to patients across diverse needs. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient conditions, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) systems Administer medications and perform basic medical tasks such as vital signs measurement, catheter care, and wound management under supervision Support patients with specialized needs such as Alzheimer's care, dementia care, hospice, and end-of-life care with patience and empathy Assist with meal preparation, feeding, light housekeeping, laundry, and maintaining a clean and safe environment Use assistive devices like Hoyer lifts or other mobility aids to ensure safe transfers and mobility assistance Document all patient interactions thoroughly in compliance with home healthcare regulations and HIPAA standards Qualifications Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring memory care Knowledge of medical terminology, medication administration, and vital signs monitoring Experience working within home health agencies or long-term care settings preferred Ability to lift heavy objects safely and perform physically demanding tasks such as repositioning patients or assisting with transfers Familiarity with EMR/EHR systems for accurate documentation Strong communication skills to effectively collaborate with patients, families, and healthcare teams CPR and First Aid certification required; Basic Life Support (BLS) training preferred Knowledge of home & community care practices, hospice protocols, dementia care strategies, and end-of-life support Join us in providing compassionate home healthcare that empowers individuals to live comfortably and confidently in their own homes.
