Duties Assist clients with activities of daily living (ADLs), including personal care, meal preparation, and medication administration Support clients during community outings, appointments, and transportation needs using appropriate patient transfer methods such as Hoyer lifts or manual handling techniques Monitor patients' vital signs and observe for any changes in health or behavior, especially in memory care or dementia-related cases Implement individualized care plans tailored to each client's unique needs and preferences Maintain accurate documentation of patient care activities, medication logs, and incident reports in compliance with HIPAA regulations Provide emotional support and companionship to promote mental well-being and social engagement Ensure a safe environment by performing routine cleaning, sanitation, and safety checks in accordance with health standards Skills Proven experience working with individuals with developmental disabilities, neurodevelopmental disorders, or aging populations such as seniors or those in hospice care Knowledge of patient monitoring techniques including vital signs, patient observation, and dementia or Alzheimer's care strategies Familiarity with assisted living environments, nursing homes, home health services, or long-term care settings Ability to operate medical equipment such as Hoyer lifts and perform patient transfers safely Strong communication skills for effective client interaction and collaboration with team members Understanding of resident's rights, HIPAA compliance, and confidentiality standards Ability to handle heavy lifting safely and perform manual handling tasks as required Certifications such as CPR/First Aid are highly desirable to ensure preparedness for emergencies Experience assisting with ADLs like bathing, dressing, grooming, and mobility support in a caring manner Competence in meal preparation tailored to dietary needs and assisting clients with personal transportation when necessary Join us as a Direct Support Professional to bring compassion into action!
