Responsibilities Assist individuals with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques and patient transfer methods such as Hoyer lifts. Monitor patients' vital signs and observe changes in health status, ensuring timely communication with healthcare teams. Support medication administration in accordance with care plans and HIPAA regulations to ensure safety and compliance. Prepare nutritious meals and assist with meal planning to meet dietary needs, including specialized diets for dementia or Alzheimer's care. Maintain a clean and safe environment by performing regular cleaning duties and adhering to infection control protocols. Provide transportation assistance for appointments, community outings, or errands while ensuring patient safety during transfers. Document care activities accurately in compliance with privacy standards such as HIPAA, updating care plans regularly to reflect evolving needs. Skills Proven experience working with individuals with developmental disabilities, intellectual disabilities, autism spectrum disorder, or neurodevelopmental disorders. Strong knowledge of patient care techniques including patient monitoring, first aid, CPR certification, and emergency response procedures. Familiarity with resident's rights, dementia care strategies, memory care practices, and hospice or geriatrics support. Ability to perform heavy lifting and manual handling safely while assisting residents with mobility or transfers using equipment like Hoyer lifts. Excellent communication skills for engaging effectively with clients, families, and healthcare professionals. Experience working in group homes, nursing homes, assisted living facilities or home health environments is highly desirable.
